Important Registration Forms

For your convenience, we have added forms online so you can complete them before your visit at home where needed information will be easier to access.

NOTE that these forms MUST BE COMPLETED BEFORE YOUR FIRST VISIT:

Once you have completed the forms, FAX them to 530.241.5677
or mail them to: Apogee Outpatient Surgery Center, 1238 West Street, Redding, CA 96001

You should receive a call within 1 business day of receipt confirming we have received your form. If you have any questions, please call (530) 241-5499.

IMPORTANT: We must receive the above forms a minimum of 3 business days before your surgery is scheduled. Please mail or fax them at least 5 days prior to your surgery and call to confirm we have received them. If we have not received your completed forms prior to surgery, you will need to complete them again.
